ACCESSION 1964/22 — WARD DAY BOOKS, 1949–1961, TWELVE VOLUMES

Received from the ward sister's office on closure of the ward.

Condition: sound. Volume 7 water-damaged at the fore-edge, legible.

Treatment: retained in both forms. The continuous copy is to be kept in the office and the originals shelved in the basement store. Where the two differ the original governs and the difference is entered on the sheet at the front of the continuous copy.

Reconciled copy checked against the originals 8.6.64 by E. Vale and countersigned by the deputy records officer.

Note: no volume is to be improved. Corrections are entered beside, not over.

Cataloguer's note (2007): the phrases “continuous copy” and “retained in both forms” are standard hospital records vocabulary of the period and appear in three other institutions' manuals held in this series.
